SPECIALIZED TESTS
POLICIES

Policies for our specialized NutriWhite exams:

Upon receiving the test quote, you have 7 days to make the payment.

NutriWhite does not handle arrangements with insurance companies. You will receive your results and, upon requested, the invoice for the exam purchased under NutriWhite's registration. Please note that none of our partner laboratories issue personalized billing.

NutriWhite will not send any sample kits without first receiving your proof of payment.

Once the sample kit is received, you have a maximum of 7 days to collect and send your sample to the laboratory.

a. If you are taking any medication such as probiotics, antibiotics, or any medication that affects the intestinal microbiome, please notify your logistics advisor to coordinate the timing for sample collection.

b. If you are undergoing a hormone test, please coordinate with your logistics advisor the timing for sample collection, considering your menstrual cycle.

c. If more than 7 days pass without collecting and sending your sample, we regret to inform you that your sample will not be processed, and your payment will not be refunded.

If you accidentally lose or discard any of your test kits, an additional fee of $30 USD will be charged to send you a new kit.

Blood tests (venous extraction) may require a medical order from your primary care doctor. NutriWhite does not cover the costs of blood sample collection (venous extraction).

If you do not reside in the United States, you can access our shipping logistics by prepaying the fee. Our logistics advisors are available to guide you through the process.

It is mandatory to complete all forms inside the kits. Example forms and instructions will be sent by email once your payment is processed.

• NutriWhite is not responsible for samples arriving at the laboratory without paperwork, as they will not be processed.

You must fill the requisition form with the exact first and last name provided to your NutriWhite logistics advisor. If the requisition form is filled with a name, nickname, or alias different from the one provided, the sample will be canceled. Only mark the tests you will be undergoing on the requisition form. If you mark any additional tests, even if you do not undergo them, the laboratory will charge for them.

The test results will be ready within 15 to 25 business days after your sample arrives at the laboratory. They will be delivered and interpreted by your NutriWhite Ambassador during the consultation. After the appointment, the results will be sent by email.Los resultados de los exámenes estarán listos en un plazo entre 15 a 25 días hábiles, una vez su muestra llegue al laboratorio. Los mismos van a ser entregados e interpretados por su Embajador NutriWhite en consulta. Posteriormente a la cita, las pruebas serán enviadas vía email.

Exam results will only be given to the person who took the test. If you would like someone you trust to receive the results on your behalf, you must send a signed formal authorization email to your NutriWhite logistics advisor.

NutriWhite is authorized to use and disclose information from the patient's history, as long as said use and disclosure (i) does not involve personal identification data, such as names, surnames, ID numbers, and home addresses; and (ii) is strictly and solely related to conducting studies and research by NutriWhite, as long as these are directly connected to NutriWhite.
